What information do I need?
In order for me to arrange an appointment I will need your contact details (name, phone number, and email address). I will also need information about your address, age and health in order for a treatment to proceed.
Why do I need it, how will I use it, and how long will I retain it?
I need all the details above so that I am able to contact you to arrange and remind you about your appointment, to ensure that it is safe for me to provide any treatment you have requested, and also for insurance purposes. I will store your information for 7 years after the date of your last treatment, or for 7 years after you have reached the age of 18 if you are/were under 18 when your treatment took place. My lawful basis for processing this personal information is 'contract' which means that I have a legal reason to ask you for your data because I need it for contractual reasons. You do not have to share your personal data with me, but if you do not then I cannot provide you with a treatment. You must fulfill your side of the contract (share your personal information) so that I can fulfill mine (carry out the treatment).
Will I share your information?
I use a MailChimp account to send marketing emails. If you agree to being contacted by email for marketing purposes I will share your first name, last name, email address and birth date with MailChimp. They will only use that information to enable me to send you marketing emails.
I use iZettle and PayPal to process non cash payments. If I am requesting funds from you online I will share your email address with PayPal purely for the purpose of obtaining payment. If you are paying by card during your appointment, I will share with iZettle either your email address or mobile telephone number (whichever you agree to at the time) purely to enable an electronic receipt to be sent to you.
Who can I complain to if I feel you are not handling my data correctly?
